The game is set in Cottonmouth, within the Grand Theft Auto universe (Liberty City, Vice City, Carcer City, San Andreas and Bullworth etc.) The main protagonist is Dr. Daniel Lamb, who volunteers to be the guinea pig for a scientific experiment codenamed "The Pickman Project". 6 years later, Daniel is a patient of a mental institution; Dixmor Hospital for the Criminally Insane, with no memory of who he is or of his past. Meanwhile, a "bridge" in his brain has been disrupted, triggering an alternate personality, Leo Kasper. Unaware that Leo is merely a figment of his imagination, Daniel escapes Dixmor with him during an electrical storm. Manipulated into murder, Daniel begins searching for the truth about the experiment, all the while trying to remember his past.
